Job Summary

This role is responsible for providing leadership and supervision in the planning, development, coordination and implementation of existing and future Adult Day Programs for seniors and adults with disabilities and the Enriching Lives through Leisure program. These programs support and enable seniors to continue to live independently and with dignity in the community.

Duties & Responsibilities

  • Supervise a diverse team of day program staff, volunteers and students, promoting team effectiveness through the provision of excellent communication strategies, adequate resources, performance reviews, education and ongoing supervision.
  • Manage three Adult Day Programs through the onsite coordinators to provide therapeutic and social recreation programs, personal support services and relief for caregivers of frail, elderly and/or disabled adults and for cognitively impaired adults living in the community.
  • Supervise the Enriching Lives through Leisure program and on-site coordinator at the John R Rhodes site.
  • Work closely with Manager of Community Support Services providing back-up as needed for the Assisted Living Program sites and staff; and on-going support with reports, business proposals, program statistics and budget information for Ontario Health reporting.
  • Monitor the operation of services for seniors and adults with disabilities and respond to needs of clients and their caregivers thought waitlist management, care coordination, up to date care plans, maintain relevant documentation, caregiver supports and referrals.
  • Submit statistical requirements and meet the mandate of Ontario Health initiatives and directions.
  • Oversee the purchasing budget and monitoring of program budgets, reporting variances to manager.
  • In collaboration with manager, meet quality management standards, contingency planning and overseeing program improvements, through the development, implementation, monitoring and evaluation of policies, procedures, services and standards for program delivery and related support functions.
  • Ensure risk management processes and Health & Safety requirements including infection control and safe food handling are met by staff and contracted supports.
  • Ensure effective case management, program delivery and response to program enquiries and complaints.
  • Perform other duties as assigned.

Skills & Qualifications

Essential

  • Possess a Diploma/Degree in Social Sciences /Gerontology/Recreation and Leisure from a recognized educational institution.
  • Strong leadership skills in order to effectively lead a team of diverse staff.
  • Extensive knowledge and expertise relating to the aging population, dementia, therapeutic programming principles, and the community support services sector.
  • Must also have exceptional interpersonal and communication skills to work with a variety of people, taking into consideration varying needs.
  • Strong relationship building skills are required to ensure successful partnering and represent Halton Region on a number of community committees.
  • Knowledge of relevant acts and legislation including the Home Care and Community Services Act, Health Care Consent Act; and the Personal Health Information Protection Act, along with working knowledge of the health and safety legislation including the Occupational Health & Safety Act.
  • Minimum of five (5) years related experience.

Preferred

  • Conceptual understanding/knowledge of Management Information Systems and mathematical skills in budget monitoring.
  • Broad-based knowledge of issues in gerontology and resulting health issues; social work principles, therapeutic skills, assessment skills; human development and behaviours.
  • An equivalent combination of education and experience.

  • Current (obtained within the past six (6) months), original and acceptable Criminal Records Check, including Vulnerable Sector Screening by the first day of employment.
  • Current two-step tuberculosis test (obtained within the past six (6) months) by the first day of employment.
