Job Primary Care Physician Manager in North York
Position Summary:
The Primary Care Manager is responsible for coordination and supervision of the Primary Care clinic and team. The Manager works closely with the Clinical Administrative Coordinator and the Director of Quality and Clinical Services (their supervisor).
The Manager is responsible for the day-to-day delivery of high quality programs and services that respond to the needs of clients. The Manager collaborates closely with team members and internal and external partners to implement the vision of primary care service delivery. In addition the Manager is responsible for budget management, and ongoing analysis and assessment of systems, structures and measurable outcomes for programs and services.
Specific Tasks:
- Conduct all aspects of recruitment, supervision and performance management of primary care team members.
- Plan, implement and monitor services that respond to client and communities’ needs, and meet internal and external performance targets
- Oversee the day-to-day functioning of the primary care clinic
- Develop, implement and ensure adherence to primary care policies, protocols and workflows
- Oversee primary care intake processes
- Working with the Data Management Coordinator, support primary care team in maximizing use of EMR
- Coordinate and co-lead team planning/meetings/training/education
- Lead the identification, implementation and evaluation of quality improvements within primary care
- Oversee the primary care team student/resident placement program.
- Working with Clinical Director, set and manage team budget and assist in pursuing funding opportunities to support and build primary care services
- Prepare and present information and reports on primary care activities
- Keep Director informed and up to date on relevant issues.
- Build and maintain effective cross-team and cross-agency collaborations to enhance primary care service delivery
- Participate in external networks and partnerships and at external events
- Oversee clinic infection control
- Coordinate team case conferencing, incident debriefing and impact debriefing
- Responds to primary care client complaints and escalations
- Act as one of a team of Health Centre critical incident responders
- Identify and work to mitigate risks and operational challenges
Job Type: Full-time
Salary: $71,174.00-$85,749.00 per year
